For my research field, probably Harvard (likely accessible through either MGH/BWH or BIDMC) and University of Washington have similarly extensive research programs, and probably 10 or so universities have several really good groups.

I don't know if I see it as a red flag. ID is relatively less competitive so knowing exactly how many people to interview to ensure you fill the spots in a buyers market could be tough. It probably fluctuates significantly by how many internal candidates are applying to the program.
I am wondering given how famous Northwestern is , why the candidates haven't ranked it ... I am sure they invited at least 3-4 people per spot! Anyway- its difficult to know without insider's info....
